Following AnchorNotes, a dated completion record would make later reporting more useful. The published URL, destination, surrounding text and agreed terms describe the deliverable. Referral visits or enquiries, where reliably attributable, describe a possible outcome. They should be separate columns.
I would keep a notes column for other changes made during the reporting period: a new landing page, a product launch, another advertisement or a technical fix. That context prevents a before-and-after chart from being treated as an explanation by itself.
An offer does not need a complicated dashboard to be clear. A small report with defined fields and a stated measurement period is often easier to review than several screenshots without dates or explanations.
